Candle McMurphy, arrestato per piccoli reati, viene portato in una clinica psichiatrica: tenta di fingersi pazzo per sfuggire al carcere. Qui viene a contatto con gli altri pazienti dei quali diventa ben presto il beniamino: si prende gioco delle sedute di psicoanalisi, si improvvisa telecronista, organizza una piccola gita in barca. L'ospedale è diretto dalla ferrea capoinfermiera, la signorina Ratched che ha organizzato la vita dei pazienti secondo regole di intransigente disciplina.
